Authorization: I hereby authorize Travel and Transport New England (Travel and Transport New England or “you”) to charge, to the credit card named below, airline tickets for myself, my immediate family, and anyone else whose tickets are ordered by me or my assistant. This authorization to charge also applies to your standard service fees, hotel deposits and guarantees as well as other travel arrangements for myself, my immediate family, and anyone else as ordered by me or my assistant. This authorization will continue in effect and renew automatically beyond the expiration date noted on the current card as the credit card is renewed/extended from year to year until I advise you in writing to the contrary. A copy of the front and back of the card is attached.

Agency: I acknowledge that Travel and Transport New England acts as a sales agent for any airline, hotel, car-rental company, tour operator, cruise line, or other service provider named in my itinerary (“Suppliers”). Travel and Transport New England is not responsible for acts or omissions of the Suppliers or their failure to provide services or adhere to their own schedules.

No Liability: I acknowledge that Travel and Transport New England assumes no responsibility for and shall not be liable for any refund, personal injury, property damage, or other loss, accident, delay, inconvenience, or irregularity which may be caused by: (1) any defaults, wrongful or negligent acts, or omissions of the Suppliers; (2) any defect in or failure of any vehicle, craft, equipment, or instrumentality owned, operated, or otherwise used or provided by the Suppliers; or (3) any wrongful or negligent acts or omissions on the part of any other party not under Travel and Transport New England' control. I hereby release Travel and Transport New England from all claims arising out of any problem covered in this paragraph.

Hazards: I acknowledge that Travel and Transport New England has no special knowledge regarding the financial condition of the Suppliers, unsafe conditions, health hazards, weather hazards, or climate extremes at locations to which you may travel. For information concerning possible dangers at destinations, I agree to contact the Travel Warnings Section of the U.S. State Department at (202) 647-5225 or www.travel.state.gov. For medical information, I agree to contact the Centers for Disease Control at (877) FYI-TRIP or www.cdc.gov/travel. I hereby assume full and complete responsibility for checking and verifying any and all passport, visa, vaccination, or other entry requirements of my destination(s), and all conditions regarding health, safety, security, political stability, and labor or civil unrest at such destination(s). I hereby release Travel and Transport New England from all claims arising out of any problem covered in this paragraph. I hereby agree that the courts in Massachusetts will be the exclusive jurisdiction for all claims brought by me or Travel and Transport New England, and I hereby agree to submit to the personal jurisdiction of those courts.

Protection : For tours and cruises, I acknowledge that Travel and Transport New England strongly recommends that I purchase trip cancellation insurance and that I check the appropriate box below. I acknowledge that, for all forms of travel, Travel and Transport New England strongly recommends that I use a credit card for my purchase, so that I can exercise my rights under the Fair Credit Billing Act if I do not receive the services purchased.

Consolidators: I hereby authorize you to purchase discount airline tickets from a broker known as a consolidator. I hereby acknowledge that: (1) consolidator tickets might not be honored if the consolidator defaults or the airline refuses to honor the tickets, (2) consolidator tickets are generally nonrefundable, non-changeable and not usable on other airlines, and (3) my credit card's automatic flight insurance may not apply to these tickets. I agree to refrain from making any claims against you arising out of any problem covered by this paragraph.

Unauthorized Ticketing: If I make a reservation for a back-to-back, hidden-city, or other unauthorized ticket, I agree to reimburse Travel and Transport New England for the amount demanded by the airline.
